CenterWell is a senior-focused primary care center designed for patients 65 and older. Our Care Team includes a doctor, nurse practitioner, medical assistants, and a referral coordinator. Patients also have access to a nurse care coach, behavioral health specialist, social worker, and clinical pharmacist. The whole team works together to meet all aspects of a patient’s health needs – physical, emotional, and social. The idea of “care centered on you” is about giving seniors the time, space, and respect they deserve.
